For your convenience Hopkins Athletic Department has made forms and resources available on line .  You can select and print any of the following forms/resources.  If you need a form or document that is not listed here please contact us and we will get it added to the website as quickly as possible.   

PARTICIPATION FEE INFORMATION

New for the 2010-2011 school year is a participation fee for all athletics.  Click on the following link for more information.  Participation Fee

PHYSICALS

Current-year preparticipation physicals are required for athletic participation.  The most commonly used form is the MHSAA physical card.  Please be advised that a current-year physical is one given on or after April 15 of the previous school year.  All current-year physicals expire July 1st. TRANSPORTATION

Parents can release their student athlete to ride home with another party other than Hopkins Public Schools Transportation.   Transportation Permission Slip

PATRON PAGE

Each season athletes are given the opportunity to fund raise for their specific team through Patron pledges.  Fall and Winter patrons will be listed in our sports program.  Patron Page
