Required for all children enrolled in 6th grade and for all children 11 through 18 years of age changing school districts:
- Meningococcal Vaccine (MCV4 or Menveo) on or after the 11th Birthday, and a 2nd dose of Meningococcal Vaccine is recommended after the age of 16.
- One dose of Tetanus/Diphtheria/Acellular Pertussis (Tdap) on or after the 11th birthday if 5 years since the last dose of Tetanus/Diphtheria Vaccine (Dtap, Td, DT)
- Two doses of Varicella (Var) or history of Chickenpox Disease
